●「考える内視鏡診断」のポイント
･本邦ではSSBEに伴うBarrett食道癌が多いので，食道胃接合部（EGJ）を深吸気下で注意深く観察する．
･表在型Barrett食道癌のほとんどは発赤調を呈していることから，まずBarrett食道粘膜内の発赤部位に着目する．
･表在型Barrett食道癌の多くが0〜3時方向（前壁から右壁方向）に認められることから，この部位の粘膜面の変化に特に注意する．
･病変を0〜3時方向に認めた場合，他部位にも病変がないかしっかりと観察する意識を持つことで多発病変を指摘できる．
･酢酸撒布にて発赤がより明瞭となり，拾い上げ診断や範囲診断に有用なことがある．
･NBI，BLIなど画像強調内視鏡（IEE）による拡大観察が質的診断および範囲診断に有用であり，胃癌の診断に準じ表面構造と血管構造を詳細に観察する．
