●「考える内視鏡診断」のポイント
1.早期胃癌の内視鏡診断のステップには,①存在診断(detection),②質的診断(characterization),③術前診断(preoperative diagnosis)がある.
2.それぞれのステップには,①技術,②知識,③経験が必要である.
3.さまざまなモダリティを臨床応用するためには,診断法の有用性に加えて限界となる病変について理解しておくことが肝要である.
4.限界例を考慮した明快なストラテジーに沿った診療clinical practiceを実践することが,医師が“考える内視鏡診断”である.
