●「考える内視鏡診断」のポイント
通常観察では
･多彩な内視鏡像が同時に観察される．
･非上皮性腫瘍としての性質（粘膜下腫瘍様）が観察され，蚕食像はなく，硬さが目立たない．
拡大内視鏡観察では
･腫瘍浸潤により腺管構造が破壊された無構造領域や異常血管が観察される．
･間質の細胞浸潤により窩間が引き延ばされ，腺管の膨化所見がみられることがある．
･前述の特徴を捉え，狙撃生検を行うことで，診断能は向上する．
･治療後に腺管構造や上皮下毛細血管の回復が観察され，治療後評価にも有用である．
