要旨●非乳頭部十二指腸腫瘍において，腫瘍径と組織学的な異型度は相関するが，粘液形質によって悪性度が異なることに注意が必要である．腸型形質を有する十二指腸腫瘍では腫瘍径30mmまでは腺腫の割合が高いが，胃型形質を有する腫瘍では10mm以下でも癌の割合が高い．十二指腸腫瘍の術前診断において，発生部位・腫瘍径に加え白色光観察，image-enhanced endoscopy併用拡大観察による総合診断が重要である．2〜3年ほど経過観察可能な病変としては，10mmまでの低異型度腺腫が挙げられるが，CSPなどの簡便で安全な内視鏡治療法の登場により，病変が小さいうちに切除し，不要な経過観察を避けるという考え方もある．
