要旨●欧米ばかりでなく本邦においてもBarrett食道腺癌の増加が報告されているが,表在型Barrett食道腺癌に対する内視鏡治療後の予後が良好なことも明らかになっている.したがって,Barrett食道腺癌をいかに早期に発見できるか,すなわちBarrett食道腺癌のsurveillanceが重要となっている.欧米では,多くのガイドラインで2〜5年に1度の内視鏡検査によるsurveillanceが推奨されているが,本邦ではsurveillanceの方法が確立していないのが現状である.しかしながら,本邦でもLSBEからの発癌率は欧米からの報告と同程度であることが報告されており,LSBEはsurveillanceの対象になると考えられる.今後,本邦におけるBarrett食道腺癌のsurveillance法の確立が望まれる.
