Xanthomatous changeを伴ったsessile serrated adenoma/polyp with cytological dysplasiaの1例

文献概要

要旨●患者は70歳代，男性．主訴は鮮血便．現病歴は，鮮血便およびCEA高値の精査のため近医にて大腸内視鏡検査を施行し横行結腸に病変を認め，精査加療目的に当科に紹介され受診となった．通常観察では，径22mmの表面隆起型病変がみられ，表面は平滑で全体に白色調変化を認めた．BLI（blue laser imaging）拡大観察で，surface patternは病変全体に円形もしくは楕円形を呈し，vessel patternは白色領域では毛細血管が比較的明瞭に視認された．インジゴカルミン色素撒布拡大観察では白色変化は窩間部に認められ，やや大きめの円形のpitを認めた．白色変化の少ない領域ではpitの一部に鋸歯状変化を認めた．総合的に鋸歯状病変を伴うxanthomaを疑い，内視鏡治療を行った．病理組織学的診断は，xanthomaを伴うsessile serrated adenoma/polyp with cytological dysplasiaであった．本稿では自験例に文献的考察を交えて報告する．
