ESDの偶発症予防策

　内視鏡治療の最難関とも言われる十二指腸ESD（endoscopic submucosal dissection）は，治療後の偶発症が多いことも大きな問題である．剝離後の潰瘍面が胆汁・膵液に曝露されることにより後出血・遅発性穿孔が高率に生じるとされており，これらの予防法を積極的に行う必要がある．最近はさまざまな偶発症予防策が発達してきており，限られた施設において十二指腸ESDを慎重に行い始めている状況である．本稿では十二指腸ESD後の偶発症予防策について概説する．
