大腸T1癌内視鏡的切除が追加手術後の再発や予後に与える影響

はじめに
　「JSCCR（Japanese Society for Cancer of the Colon and Rectum）guidelines 2019」（以下，JSCCRガイドライン）^1）においては，大腸T1癌に対する内視鏡的切除後（endoscopic resection ; ER）のpT1癌の治療方針について，病理組織学的検索にて，①T1b（SM 1,000μm以上），②脈管侵襲陽性，③低分化腺癌，印環細胞癌，粘液癌，④浸潤先進部の簇出（budding ; BD）2/3，のうち1因子でも認めれば追加治療としてリンパ節郭清を伴う腸切除を弱く推奨する，としている．しかしながら，一次治療が手術（primary surgery ; PS）の場合に比べ，追加外科的切除（secondary surgery ; SS）を行うべき症例において，ERが先行して施行され，その後，SSとなった場合に，遠隔転移を含む長期予後に影響を与えるかどうかについては明らかではなかった．一次手術（PS）群と内視鏡的切除後追加外科的切除（ER＋SS）群の長期成績の比較について，近年複数の論文が報告されている．本稿では，ER＋SSにおける長期予後に関連する文献について紹介するとともに，おのおのの臨床的背景や解析方法の違いについても注目し解説を試みたい．
