化学放射線療法後の局所再発に対する治療戦略—PDT（photodynamic therapy）を中心に

要旨●化学放射線療法（CRT）は，局所進行食道癌に対する根治的治療の重要な選択肢である．しかし，CRTを受けた患者の30〜40%において局所遺残・再発が認められる．従来，救済治療として外科的切除や内視鏡的切除が行われているが，適応症例が限られている．光線力学的療法（PDT）は，腫瘍親和性光感受性物質を用い，その吸収波長に一致したレーザの照射により生じる光化学反応を利用して腫瘍細胞を破壊する局所治療である．CRT後またはRT後局所遺残・再発食道癌に対する救済治療として，タラポルフィンナトリウムを用いたPDTが2015年から保険適用となっている．本稿では，PDTの適応および実際の手技，治療後の管理について具体例を提示し説明する．
