要旨●潰瘍性大腸炎の治療目標として,内視鏡的粘膜治癒と同様に組織学的治癒の重要性が認識されるようになった.しかし,内視鏡的活動性と組織学的活動性には乖離を認めることがあり,組織学的治癒を効率的に評価できるような新たな画像認識技術が求められている.これまで内視鏡的粘膜治癒の判断には白色光観察による活動性スコアリングが用いられてきたが,近年のIEE技術の発展により,白色光観察では捉えられない微細な粘膜の炎症性所見が認識可能となった.IEE観察による内視鏡と組織学的活動性診断の相関が検証できれば,IEEは内視鏡的炎症の診断に必要不可欠な技術となると考えられる.
