食道の扁平上皮内腫瘍

はじめに—歴史
　最初に，食道の上皮内病変について歴史的な記述を見てみたい．StoutとLattesの食道の腫瘍病理学書^1）（1957）では食道の前癌病変として，leukoplakiaとcarcinoma in situを挙げて説明している．次に，中国河南省の食道の扁平上皮癌の好発地域で行われた細胞診によるマススクリーニング^2）（1959年〜）では，前癌病変についてdysplasia（異形成）を挙げている．また，Ushigomeら^3）（1967）の症例報告があり，Mukadaら^4）（1976）と筆者ら^5）（1981）は，剖検食道からdysplasiaを探し出し，食道癌例では頻度が高いことを報告している．しかし，これらの論文の中では，dysplasiaと正常上皮との上皮内の境界に，腫瘍を示唆するoblique lineを見いだす努力がなされておらず，dysplasiaは異型上皮（atypical epithelium）と同義的に用いられている．
