好酸球性食道炎

　好酸球の消化管局所への異常集積によって組織が傷害されて機能不全を起こす疾患群は好酸球性消化管疾患と称され，部位により好酸球性食道炎（eosinophilic esophagitis ; EoE），胃炎（eosinophilic gastritis ; EG），胃腸炎（eosinophilic gastroenteritis ; EGE），大腸炎（eosinophilic colitis ; EC）に大別される．
　これらのうち，EoEは，病因は明らかになっていないが，アレルギー反応により食道へ好酸球が浸潤して慢性炎症を起こし，食道運動障害や食道狭窄を来す疾患と考えられており，男性に多い．本邦における健診内視鏡検査での有病率は0.4%で，上昇傾向にある可能性が指摘されている．本邦では厚生労働省研究班より診断基準が示されており，食道機能障害に起因する症状と，食道粘膜の生検で上皮内に好酸球数15以上/HPF（high-power filed）が必須所見とされている^1）．
