胃癌のリスク層別化

リスク層別化の目的
　“層別化”とは，対象を特徴によってグループ分けすることで，医療では各グループに対して適切なマネジメントを行う目的で行われる．この概念が胃癌対策において重視されるようになった理由は，本邦のH. pylori（Helicobacter pylori）感染率が低下し，日本人の胃癌リスクが変化しているためと考えられる．病院受診患者のH. pylori感染率は1970年代に74.7%であったが，2010年代には35.1%にまで低下している．また，それに伴って胃癌発生の高リスク因子である胃体部粘膜の萎縮と腸上皮化生の頻度は，1970年代にはそれぞれ82%と32%であったものが，2010年代には19%と4.7%にまで低下している^1）．日本人は以前のようにすべてが胃癌の高リスク群ではなく，高リスク群と低リスク群が混在した集団であることがわかる．したがって，患者をリスク別にサブグループに分けて，適切な間隔でのサーベイランス内視鏡を推奨することは胃癌の予防対策を効率化する（Fig.1）．
