NHPH—敵か味方か不思議な細菌

はじめに
　本特集号にはNHPH（Non-Helicobacter pylori Helicobacter）の最新の知識が収載されている．NHPHが感染率の低下や除菌が進むH. pylori（Helicobacter pylori）に取って代わる胃炎の主役になるのか，あるいは脇役か，本特集号を熟読いただければ結論が出るであろう．胃内細菌の存在については1800年代の後半から指摘されていたが，分離培養ができなかったことから病的意義については疑問視されていた^1）．しかしながら，1982年のWarrenとMarshall^2）のH. pyloriの歴史的発見により，胃粘膜に細菌が感染し，持続的な炎症を起こすことにより萎縮性胃炎や消化性潰瘍，さらに胃癌の発生に関与していることが明らかになった．
　1980年代後半からヒトの胃に生息するH. pylori以外の，Gram染色陽性の大型の螺旋菌が発見され^3），当初はGastrospirillum hominisと命名され，その後遺伝子解析の結果，H. heilmannii（Helicobacter heilmannii）に再分類され，さらに，H. heilmanniiはブタを自然宿主とするH. suis（Helicobacter suis）を代表とするType 1と，イヌとネコの胃に生息するH. felis（Helicobacter felis）などのType 2に分類されている^4）．NHPH発見の歴史や分類については，本邦の，いや世界の第一人者である中村正彦氏の論文に詳述されている．最近では次世代シーケンサーの開発により，胃内にはHelicobacter属だけでなく，多くの細菌が生息し，胃癌をはじめとして上部消化管の疾患の発生に関与していることが指摘されている^5）6）．
　NHPH診断の第一歩は上部消化管内視鏡検査（esophagogastroduodenoscopy ; EGD）である．H. pylori陰性の鳥肌状胃粘膜，胃体部にRAC（regular arrangement of collecting venules）が明瞭で萎縮は認めず，胃角部周囲にびらんや発赤，褪色粘膜を認めるなどの，H. pylori感染ではみられない内視鏡所見が発見の契機になる（Fig.1）．しかし，内視鏡検査で正常に見える症例も存在するようである^7）．NHPH感染の内視鏡像については，この分野の先駆者である塚平俊久氏の論文に詳述されている．
　NHPH感染の病理組織像（Fig.2）については，当初は炎症や萎縮，腸上皮化生はH. pylori感染と比較すると軽度であるとされていた．しかし，胃癌との関連を考えると，萎縮や腸上皮化生の発生とその程度が重要であるので，現状はどのようになっているのか太田浩良氏の論文が注目される．
