要旨●家族性大腸腺腫症（FAP）に対する最近の内視鏡に関係する研究，大腸ポリープや十二指腸ポリープを内視鏡的に積極的に摘除する治療法を紹介した．大腸ポリープを内視鏡的に積極的に摘除し，大腸癌予防を目指す研究も多施設研究で安全性と5年間の進行癌予防効果が示され，2022年度から本治療（intensive downstaging polypectomy ; IDP）が保険収載された．大腸切除をすることなくIDPにより大腸癌が予防できるかどうかを明らかにするため，長期追跡のためのレジストリが準備中である．FAPにおける非乳頭部十二指腸ポリープに対するIDPの成績についても紹介した．また，大腸未切除FAP患者に対する低用量アスピリンの8か月間投与による二重盲検試験において大腸ポリープの増大を予防する効果が示され，現在は，2年間の長期投与試験が進行中である．適切な内視鏡治療介入および低用量アスピリンによる化学予防でFAP患者における腸管手術を回避できる時代が近づいてきている．
