十二指腸濾胞性リンパ腫—白色絨毛との鑑別を中心に

要旨●患者は70歳代，男性．胃癌術後の経過観察目的に施行したEGDで，十二指腸下行部に顆粒状隆起が集簇していた．色素撒布で隆起および結節の大小不同が明瞭となり，さらにNBI拡大観察では絨毛の腫大と絨毛内部の不透明な白色物質，および拡張した血管が観察された．同部位からの生検で上皮下の間質に小型異型リンパ球の濾胞様増殖を認め，免疫組織化学染色からGrade 1の濾胞性リンパ腫（FL）と診断された．十二指腸下行部に白色顆粒状隆起を認めた場合は，FLを念頭に置いて白色絨毛を呈する疾患の鑑別を進める必要がある．
