非乳頭部十二指腸上皮性腫瘍に対する超拡大内視鏡診断（endocytoscopy）の有用性

要旨●非乳頭部十二指腸腺腫・癌の統一された内視鏡的な診断基準はいまだない．加えて，十二指腸は生検の診断精度と，生検による治療時の難度の上昇が課題である．超拡大内視鏡観察で術前診断ができれば組織学的侵襲がないために意義は大きいと考えられる．自験例における超拡大内視鏡観察の術前診断精度を検討したところ，その正診率は89.6%であった．本稿では非乳頭部十二指腸上皮性腫瘍に対する超拡大内視鏡診断の現状を記す．
