拡大内視鏡を施行したセリアック病の1例—十二指腸粘膜の内視鏡所見を中心に

文献概要

要旨●患者は60歳代，女性．20XX−1年6月頃から10行/day前後の水様性下痢と体重減少を認めていた．20XX年2月に原因精査目的で当院へ紹介され，消化管検査を施行したところ，十二指腸から上部小腸を主体としたびまん性の絨毛萎縮を認め，病理組織学的にもセリアック病に矛盾しない所見であった．診断に際しては，特に拡大内視鏡による微細な観察が有用であった．グルテン除去食開始後，下痢は改善した．その後の経過で節性のperipheral T-cell lymphoma（not otherwise specified）を併発した．今回，筆者らは，セリアック病に特徴的な十二指腸粘膜の拡大内視鏡が診断に有用であった1例を経験したので，文献的考察を加えて報告する．
