咽喉頭表在癌に対する局所切除と治療成績—ELPSの立場から

要旨●頭頸部表在癌の治療として彎曲喉頭鏡を用いて咽頭を広く展開し切除するELPS（endoscopic laryngo-pharyngeal surgery）は本邦で開発され，多くの施設で行われている．この方法は表在癌を一括切除できることから良好な治療成績につながっている．一方，術中の出血のコントロールに難渋することや，術後出血対応をしなければならないことが課題としてある．さらに，術後気道の状態把握も大切であり，喉頭浮腫が懸念される場合には気管切開術や翌日までの挿管管理も必要となる．頸部リンパ節転移の頻度は少ないものの，生命予後に影響することから確実な対応が望まれる．また，初回の治療経過が良好であっても，異時性に発癌することが少なくなく，長期に経過観察することが必要となる．
