自己免疫性胃炎を背景とした胃癌の臨床病理学的特徴

要旨●自己免疫性胃炎（AIG）は胃癌の発生母地となり，H. pylori感染の減少に伴い，胃癌の背景疾患としての重要性が相対的に高まってきている．自験AIG 194例について遡及的に検討を行ったところ，胃癌合併は36例（18.6%）に47病変認められた．胃癌合併例と非合併例を比較すると，胃癌合併例のほうが高齢で，内視鏡所見では固着粘液陽性率が低く，生検標本のupdated Sydney systemによる評価で前庭部腸上皮化生スコアが高かった．AIG合併胃癌の特徴は0-IIa型の早期癌で，胃型形質の分化型腺癌が典型例と考えられた．AIGには胃腺腫も26例（13.4%）に認められ，胃癌のうち4例4病変は胃腺腫として経過観察していた症例が悪性化したもので，腺腫にも注意を要すると考えられた．
