要旨●食道上皮下病変（subepithelial lesions ; SELs）とは，“粘膜固有層以深に主座を置く病変で，かつ背景粘膜と同様の非腫瘍性重層扁平上皮に概ね被覆された病変”を意味する．SEL所見を呈する病変には上皮性腫瘍，非上皮性腫瘍，腫瘍様病変まで含まれ幅広い．さらに上皮性腫瘍には原発性・転移性，非上皮性腫瘍には粘膜固有層〜固有筋層までの各層を発生点とした多種多様な腫瘍が含まれる．どのような病変が生じうるかを把握することは適切な診断や臨床的対応のために重要である．SELの表層は非腫瘍性重層扁平上皮に被覆されていることから，内視鏡的・肉眼的には質的診断が困難なことが多く，病理組織診断の役割が大きい．
