十二指腸神経内分泌腫瘍（NEN）の病理組織像

要旨●十二指腸に発生する神経内分泌腫瘍（NEN）は，WHO分類および産生ホルモンで分類すると理解がしやすい．十二指腸NENはNETがほとんどで，球部に多いガストリン産生腫瘍，乳頭・副乳頭部に多いソマトスタチン産生腫瘍，十二指腸遠位に多いセロトニン産生腫瘍，乳頭部付近に発生するgangliocytic paragangliomaがあり，それぞれで病理組織学的所見に多少の差異がある．予後はいずれも良好である．内分泌細胞癌（NEC）はまれで，乳頭部に多く発生し，予後が著明に不良である．十二指腸NENを治療する際は，その病理組織学的所見を理解する必要がある．
