Rindi分類に含まれない胃神経内分泌腫瘍（NET）の疾患概念

要旨●1993年のRindiらの報告以来，胃神経内分泌腫瘍（NET）は疾患背景によってType I〜IIIに分類されてきた．しかし近年では，いずれにも分類できない特徴を持つ疾患群が報告されている．徐々に報告が増えてきている壁細胞機能不全症を背景とするものや，プロトンポンプ阻害薬（PPI）やカリウムイオン競合型酸阻害薬（P-CAB）の長期内服によるものが挙げられる．いずれも長期にわたる酸分泌低下あるいは抑制による高ガストリン血症が原因で，NETが発生すると考えられる．壁細胞機能不全症では疾患自体の正確な診断，PPIやP-CABの長期投与症例では薬剤継続の必要性を見極めつつ，継続が必要なら，両者ともに内視鏡を含めた定期的なフォローアップが必要であろう．
