リウマチの破骨因子

■新たなリウマチの破骨細胞分化誘導系

　関節リウマチ（RA）の罹患者は日本国内で約80万人ともいわれ，これらの患者の機能障害をいかに軽減するかは医療福祉上・医療経済上重要な問題である．RAは四肢の関節破壊を主徴とするが，これに伴って加齢によるものよりはるかに高度な骨粗鬆症が生じ，これも患者の機能を大きく損ねる要因である．しかし高度な骨粗鬆の病態は解明されていない部分が多く，その予防・治療の方法はまったく確立されていないといっても過言ではない．

　RAでは関節腔内からも骨破壊が進行するのが特徴的である．関節滑膜組織内に破骨細胞様の形態をとる多核巨細胞が存在することは以前より知られており，これが関節内からの骨吸収に関与する可能性も示唆されていた．われわれはRA関節腔内に前駆破骨細胞が存在することを予測し，RA患者関節液中に大量のCD14陽性単球様細胞の存在と，これらがサイトカインの刺激により骨吸収能を持つ成熟破骨細胞に分化することを見出し，前駆破骨細胞であるCD14陽性単球様細胞は，健常人末梢血単球とは異なる表面抗原プロファイルを有しており，分化段階の異なる細胞であることが示唆された^5）．
