頚椎後側弯変形を伴う四肢麻痺を呈したneurofibromatosis type 1の1例

　頚椎後側弯変形を伴ったneurofibromatosis type 1（NF-1）に対して後方除圧固定術を行った1症例を経験したので報告する．症例は35歳，男性．巧緻運動障害，四肢体幹のしびれ，歩行困難が出現し，当科を受診した．単純X線画像上では，C5頂椎の後側弯変形を認め，MR画像では頚椎C5-7での椎体前方の腫瘍性病変と後弯変形による脊髄の圧迫を認めた．手術は前方の腫瘍性病変がみられたため，後方からのC4-7椎弓形成術，Summit PCRを用いた後頭骨からT2までの脊椎固定術を選択した．術後，開口制限と嚥下困難が残存した．そのため，追加手術を施行した．手術は後頭骨プレートを抜去し，ロッドをC3上方で切離した．本症例は脊柱変形が著しく，前方後方法の適応であったと考えられたが，椎体前面の腫瘍性病変が存在したため後方法を選択した．現在，術後3年8カ月が経過しているが，開口障害は消失し，日常生活における障害もなく経過している．
