変形性膝関節症における血清中の関節マーカーの有用性―血清cartilage oligomeric matrix protein（COMP）値とヒアルロン酸（HA）値について

　関節マーカーは軟骨や滑膜などの関節構成体の代謝に関連する因子を関節液，血液，尿などの体液中で測定することにより関節構成体の合成や破壊の動態を知るものである．近年，血清中で測定可能なマーカーであるcartilage oligomeric matrix protein（COMP）やヒアルロン酸（HA）などの有用性が報告されている．われわれの検討では変形性膝関節症患者では血清COMPはX線病期に応じて変動を示し，HAは臨床症状に一致した変動を示した．マーカーは現時点では，変形性膝関節症診断の補助的手段とみなされているが，複数のマーカーを組み合わせることによって，進行の予知，より正確な病態判断も可能と考えられる．合併する脊椎椎間関節症などの影響も考慮する必要があるが，血清マーカー測定は関節穿刺を必要とせず，侵襲の少ない変形性膝関節症における病態評価法として期待されている．
