変形性膝関節症：高位脛骨骨切り術の進歩―Opening Wedge法による高位脛骨骨切り術

　高位脛骨骨切り術（HTO）は，変形性膝関節症の外科的治療の基盤をなす術式である．しかし，良好な術後成績を得るには経験と手術技量が求められ，長期で複雑な後療法が必要となる．医療の効率化と在院日数の短縮が求められる現在，これらは克服すべき課題である．これらの問題を克服するには，術式の単純化と骨切り部の強固な固定が必要となる．最近，locking screwを用いる内固定材（TomoFix）が開発され，opening wedge法によるHTOが再び着目されるようになった．TomoFixと術中矯正位の保持と開大部の骨癒合の促進を目的に楔状ハイドロキシアパタイトを用いたopening wedgeの術式を行った．術後の下肢長不等や膝伸展機構への影響，矯正角度の限界，脛骨後傾の増強などの問題はあるが，術中操作と適応の厳格化により解決でき，本術式は手術侵襲が少なく，後療法の簡素化と加速化を可能とし，有用な手術術式である．
