変形性膝関節症：人工膝関節置換術のコツ

　人工膝関節置換術（total knee arthroplasty，TKA）は人工関節の材質の向上，関節のデザインの進歩，手術器具の改良，手術手技の向上により，近年，長期的に安定した良好な術後成績が得られるようになった．変形性膝関節症において理学療法，装具療法，薬物療法などで対処しきれない膝関節の内反変形や，X線上にて関節面の骨欠損，骨破壊，関節裂隙の消失した場合など，65歳以上の高齢者であればTKAの適応となる．TKAに際し，適応と禁忌を十分に理解し，術前適応の有無を検討し，全身の合併症のチェックを行い，安全に手術を行うことが最も重要である．TKAの手術に際し，TKAの術式について十分な知識を持つ必要があり，個々の患者の膝関節の状態に合わせ，適切な機種の選択を行う．手術手技のポイントとして，正確な大腿骨，脛骨の骨切り，適正な靱帯バランスの獲得，正しい下肢アライメントの獲得が重要である．また，術者は術後長期の経過観察を行う必要がある．
