外傷性頚部症候群の画像診断

　外傷性頚部症候群の多彩な臨床症状の病態解明に種々の画像診断法が用いられている．単純X線は基本的な画像検査であり，骨傷の有無のスクリーニングに不可欠である．X線側面像で明らかとなる頚椎弯曲は病的意義を持たないことが多い．MRIは軟部組織の描出に有用であり，靱帯損傷が描出される場合もあるが，稀である．椎間板変性所見は多くの場合，急性の外傷性変化ではなく加齢変化を反映している．以上のことから，急性期の本症に対するMRIの診断的意義は限定的と考えられる．認知障害などの脳障害の把握に，最近ではsingle photon emission computed tomography（SPECT）やポジトロン断層法（PET）などが用いられているが，まだその診断的価値については明らかではない．

　今後，外傷性頚部症候群の病態解明における種々の画像検査法の診断的意義を改めて検討していく必要がある．
