腰椎椎間板ヘルニアに対する顕微鏡視下椎間板ヘルニア切除術

　腰椎椎間板ヘルニアでは多くの臨床的研究ならびに治療法が報告されており，脊椎疾患でも最も関心の高い病態である．手術的治療では椎間板内療法あるいは経皮的手術やそれに類似する方法，部分椎弓切除によるLove法や顕微鏡視下椎間板切除などのstandard，内視鏡下ヘルニア切除術，など多くのものが報告されている．手技的に確実な効果が得られ，また低侵襲性でありcost effectiveであるとの視点に立てば，治療法として，Love法や顕微鏡視下椎間板ヘルニア切除術（microdiscectomy）および内視鏡下ヘルニア摘出術が重要である．顕微鏡視下椎間板ヘルニア切除術は皮切範囲の減少，神経根やヘルニア周囲の視認性，神経根への愛護性，加えてcost effectivenessといった点で優れたものと考えられる．
