関節マーカーによるヒアルロン酸注入療法の有効性予測

　ヒアルロン酸（HA）の関節内注入療法は，本邦では約20年の臨床経験が蓄積されており，膝OAに対する代表的な保存療法の1つである．HA特有の副作用は少ないが，関節穿刺という侵襲を伴うので，どのような患者に有効性が高いかは臨床上，重要である．また本法による臨床効果の改善率が30％以下のnon responderは全体の34％ほど存在するので，無駄な医療行為を避けるためにも本法の有効性を予測することが必要である．注入前の関節液中マーカーと臨床症状改善の関係を検討したところ，注入前のHA結合型アグリカン濃度と1カ月後の膝スコアの改善度は正の相関を認めた．この結果は軟骨が残存し，その軟骨組織が活発に代謝している症例で本法の有効性が高いことを示している．血中マーカーであるcartilage oligomeric matrix protein（COMP）と臨床症状改善の間には有意な関係は認められず，HA注入療法の有効性予測には関節液マーカーが血清マーカーより有用であった．
