Charnley式ソケットの手術手技

　セメント固定によるCharnley式ソケットの術後成績を向上させるため，これまでに様々な手術手技上の工夫が行われてきた．現在重要と考えるソケット固定の手術手技はジェット洗浄，臼蓋海綿骨からの止血などの臼蓋骨母床の処置に加え，過度の内方掘削を避けて外板を残し，軟骨下骨を温存し，多数の小アンカー孔，flanged socketの使用と寛骨臼によるflange部の完全被覆，セメントの高圧注入，骨囊腫の掻爬と同部位への海綿骨移植，臼不全股に対する臼蓋外側部への骨移植などである．これらの手技により長期成績が改善することを期待できる．
