Impaction bone grafting（大腿骨側）に思うこと

　人工股関節置換術（THA）の再置換の際，骨の再獲得が問題となる．Impaction bone grafting（IBG）は，骨の再獲得を目的とする有用な手術法である．現在まで112関節にIBGを行ったが，極力ロングステムの使用を避け，またロングステムからレギュラーステムへの再置換も可能であった．しかし，菲薄化した大腿骨にbone chipを強く打ち込むため骨折やステム先端の菲薄化した部での骨折が危惧され，プレートの補強を考慮しておかなければならない．骨萎縮の強い症例にも対応でき，再置換術の1つの方法として考慮に入れてよい方法と考える．
