セメントとセメントレスの適応と使い分け

　Charnley以来の人工股関節置換術はpolymethylmethacrylate（PMMA）によるセメント固定が主流であった．現在ではポリエチレン磨耗粉による骨溶解がインプラントの弛みに多大な影響を及ぼすとされているが，一時はcement diseaseといわれ，ポーラス加工，ファイバーメタル，trabecular metal，ハイドロキシアパタイトコーティングなど，セメントレス人工股関節の発展に寄与した．また，一体型のインプラントの場合，再置換時にすべて交換というジレンマもあったが，現在ではセメントレスインプラントがmodularとなっているため，摺動面のみの再置換が可能となっている．筆者らはこの利点を生かすために，骨母床のしっかりとしている条件下にて，90％の症例に臼蓋，大腿骨側ともにセメントレスインプラントを用いてきた．しかし，骨粗鬆症，廃用性骨萎縮症例，またstove pipe canalといわれるDorr type Cの大腿骨に対しては全例セメントステムを用いてきた．また，臼蓋側の再置換術の際にはimpaction graftを併用したreimforcement ringを用い，All PE（超高分子量ポリエチレン）をセメント固定して再建を行うことを基本としている．これらの適応について症例を紹介しながら述べる．
