誌上シンポジウム 発育期大腿骨頭の壊死性病変への対応

思春期の高度圧潰広範囲大腿骨頭壊死に対する大腿骨頭高度後方回転骨切り術

　治療が困難である思春期の高度圧潰広範囲大腿骨頭壊死10関節に対して大腿骨頭高度後方回転骨切り術を施行した．手術時年齢は平均14.6歳であり，急性すべり症後6関節，頚部骨折後2関節，ステロイド多量投与後が2関節である．後方回転角度は平均126°であり，平均20°の内反を加えた．術後経過観察期間は2～9年（平均4.2年）であり，最終観察時X線正面像では全例再圧潰はみられず，関節裂隙は保たれており，術前裂隙狭小化の5関節では改変がみられ，内側に移動した圧潰壊死域は再球形化がみられた．本症に対する本術式は，良好な修復が生じる有用な手術療法と考えた．
