　外傷性肩関節脱臼に対する治療に関して，最近，2つの重要な変化が起こっている．

　第1は徒手整復後に下垂外旋位で外固定する方法が広まりつつあることである．従来の内旋位固定では2年以内に56％，5年以内に87％に再脱臼が生じ，とくに10歳台男性では75％以上が再発すると報告されている^9）．一方，下垂内旋位よりも外旋位のほうが，剝離した関節唇と関節窩縁との接触がよいことは実験的に^3,8），またMRI^4,7）や関節鏡^2）でも確認されている．Itoiら^5）の無作為臨床試験（198例）によれば，内旋位固定群では脱臼再発が42％であったのに対し，外旋位固定群の再発は26％であった．外旋位固定法は逆転の発想と言ってもよい画期的なものであるが，それでも一定の率で脱臼が再発したり，外旋位固定後にもBankart損傷の修復が得られない場合がある．筆者は2007年に日本肩関節学会を主催した折，ウェブ上で会員にアンケート調査を行ったが，回答した162名のうち，初回脱臼の徒手整復後に内旋位固定（DésaultまたはVelpeau肢位）をする者が51％，外旋位固定をする者が36％であった．外旋位固定法が今後定着するかどうかは，ひとえに再発予防効果が良好か否かにかかっている．この誌上シンポジウムでは，外旋位固定に関する重要な知見が報告されており，本法の研究をさらに深めていくうえで参考になるであろう．
