誌上シンポジウム 外傷性肩関節脱臼

若年者初回外傷性肩関節前方脱臼症例に対する外旋位固定の経験―MRIまたは関節鏡所見の検討

　初回外傷性肩関節前方脱臼12例（男性11例，女性1例，平均年齢23.7歳）に対し外旋位固定を施行した．12例中8例で前方不安定性の残存が示唆され結果は不良であった．MRIおよび鏡視所見から，前方関節唇の整復位と予後との関連性は低く，前下上腕関節靱帯が発達不良の場合，外旋位固定の効果は乏しいと考えられた．装具の費用や患者への負担を考慮すると，外旋位固定の適応については今後さらなる検討が必要と考えられた．
