初回外傷性肩関節前方（亜）脱臼後の鏡視所見と手術治療成績

文献概要

　若年者では肩関節初回前方（亜）脱臼後に高率で再発することが知られている．再発の原因は初回（亜）脱臼時に生じた肩関節前方支持機構の破綻による前下関節上腕靱帯（AIGHL）の機能不全である．鏡視上では前方関節唇が関節窩前縁から剝離転位するBankart病変がみられることが多いが，前方関節包実質部での断裂や上腕骨側の関節包断裂（HAGL lesion），関節包の弛緩なども存在する．初回（亜）脱臼の治療の原則は保存治療であるが，われわれは症例を選択し鏡視下手術（Bankart修復術）を行い長期にわたり良好な成績を収めており，手術治療は初回肩関節（亜）前方脱臼の治療法として有用であると考えられる．
