Kienböck病による変形性手関節症に対する腱球移植術

　Kienböck病においては徐々に月状骨の圧壊が進んで分節化し，最終的には変形性手関節症を呈する．このようなstageにおいては月状骨の切除が余儀なくされるが，われわれは腱球移植術によって対処してきた．今回対象とした症例は25例で，全例月状骨を摘出し，腱球移植術を行った．さらにcarpal height（手根骨の高さ）を維持するために仮部分手関節固定術を24例に対して施行した．Lichtman分類ではstage Ⅲbが12例，stage Ⅳが13例であった．術後手関節痛は11例で消失し，14例で軽快した．手関節掌背屈域および握力も大半の症例において術後改善を認めた．X線計測による術前のcarpal heightは0.47であったが，術後平均は0.46と少し低下を示した．このように進行期のKienböck病に対する腱球移植術はいわゆる関節形成術として一定の効果が期待できた．ただし術後のcarpal heightの低下が問題となるが，仮固定術を併用することによってある程度carpal heightを維持することが可能であった．
