橈骨遠位端骨折後の手関節傷害の治療

　橈骨遠位端骨折の治療において関節面の不適合は変形性関節症を引き起こす．しかし関節面の大きな転位を有する症例では，初回手術時に関節面の観血的整復術がなされていたためか二次手術症例は少なかった．橈骨遠位端骨折の後遺障害として橈骨短縮による尺骨衝突症候群，遠位橈尺関節の脱臼，不安定性による遠位橈尺関節傷害に大きな問題点がある．短縮変形を残さないようにプレート固定，骨移植が必要である．受傷時，初回手術時に遠位橈尺関節の不安定性には十分な注意が必要である．
