誌上シンポジウム 手・肘関節鏡手術の現況と展望

橈骨遠位端骨折に対する鏡視下手術の実際―特に掌側プレート固定との併施を中心に

　橈骨遠位端骨折に対し掌側からのロッキングプレート固定が主流となりつつある昨今，鏡視下手術の併用はその煩雑さから敬遠されつつある．今回，関節面の正確な整復，軟部組織の損傷と処置において威力を発揮する鏡視下手術をより簡略化して導入するためにplate presetting arthroscopic reduction technique（PART）を考案し，その手技を詳述した．54例において，イメージ下に整復が得られても鏡視下に転位を認めたのは25％，舟状骨月状骨間靱帯損傷は35.2％，三角線維軟骨損傷は68.5％に合併しており，鏡視下手術併用の重要性を改めて認識すべきである．
