胸椎後縦靱帯骨化症に対する間接的後方除圧矯正固定術―術後成績は予測できる

　後弯矯正を広範囲椎弓形成とインストゥルメントを使用して行う一期的後方除圧矯正固定術は，間接的な脊髄除圧が可能であり比較的安全な手技といえる．術中の除圧操作や，除圧後に生じる後弯進行によって，脊髄障害が生じていることが術中の脊髄モニタリング（CAMP）で明らかとなった．術中脊髄モニタリング悪化を認めた群の術後成績は認めなかった群と比較して有意に悪かった．易損性の脊髄に永久的な障害を加えないよう術中CMAPは必須であり，低下を認めた場合は早急に対処しなければならない．
