誌上シンポジウム 胸椎後縦靱帯骨化症の治療―最近の進歩

胸椎後縦靱帯骨化症に対する後方進入前方除圧術の治療成績と成績不良因子に関する検討

　脊椎後方進入前方（全周性）除圧固定術は，胸椎後縦靱帯骨化症（OPLL）に対するきわめて合目的な術式であるが，同時に麻痺増悪などの危険性も高いことが報告されている．当科では2003年までに30例に本術式を行い，JOAスコア改善率平均49.5％と良好な成績を得ているが，術直後の麻痺増悪を10例に認めた．悪化した麻痺は徐々に改善することが多かったが，最終的に4例の麻痺は術前以上には改善しなかった．これら成績不良例の病態，経過について詳しく検討したところ，5椎体以上の全周性除圧が有意な危険因子であった．このことから，広範囲の胸椎OPLLの場合でも，後方進入全周性除圧は嘴状骨化巣など限局的な範囲にとどめるべきと考えられた．
