人工股関節再置換術後の大腿骨骨折に対する治療方法

　骨融解や弛みを来した骨質が不良な状態で再建を行う再置換術（revision THR）においては，術後にステム周囲で骨幹部骨折を来した場合，初回THR後の骨折以上にその治療には難渋する．Vancouver分類Bでは，プレート＆スクリュー，Mennenプレート，boneプレートなどいくつかの固定方法が報告されているが，いずれの方法においてもステムが存在する近位部分の固定力が問題となる．cable & plate systemはスクリューの刺入が困難な場合にケーブルによる固定が可能となる有用な方法であるが，やはり回旋固定に関しては不安定であり，可能であれば少なくとも，1本でも追加スクリュー固定を行うべきであると考える．また，弛みを生じている場合には，理想的には一期的に再置換を考慮することが望ましいが，まず1次的に骨接合術を行い，2次的に再々置換術を考慮することも必要である．各種の固定方法との比較についても論述する．
