誌上シンポジウム 腰椎変性側弯の治療選択

Cobb角50°以上の腰椎変性側弯症に対する手術成績―術式選択と固定範囲に関する考察

　Cobb角50°以上の腰椎変性側弯症11例に対する手術成績を調査した（追跡期間：平均3年8カ月）．矯正術を選択しなかった2例で術前の下肢痛は改善し，矯正固定術を選択した9例中7例で腰痛が改善した．側弯矯正率は，前方法3例と後方法3例では術直後平均67％，78％と良好な矯正を獲得できたが，追跡時には平均35％，67％と矯正損失を認めた．一方，前方・後方法3例では平均66％を追跡時まで維持していた．合併症は矯正固定術を選択した9例中6例に認め，前方固定術後早期に後方からの固定延長を2例に要したが，その他に経過観察中に再手術を要した症例はなかった．
