特発性側弯症に対する胸椎椎弓根スクリュー法の短期成績―従来法との比較検討

　目的：特発性側弯症に対する胸椎椎弓根スクリュー（PS）法の手術成績を調査し，フック，サブラミナーワイヤリング，PSによるいわゆるハイブリッド法と比較したので報告する．　対象および方法：free hand法によるPS法を行った20例（男5例，女15例，平均年齢13.8歳）を対象とした（PS群）．検討項目は術前後Cobb角と矯正率，調査時SRS-22スコア，合併症とした．本法導入以前に行っていたハイブリッド法による手術例12例を比較対照とした（C群）．　結果：主カーブの術前平均Cobb角はPS群65.8±7.5°，C群63.5±9.7°，調査時Cobb角18.4±8.2°，28.3±9.0°，術後矯正率72.9±10.9％，62.3±9.3％であり，矯正率はPS群で有意に良好であった（p＝0.01）．矢状面，冠状面バランスは両群ともほぼ良好に保たれていた．SRS-22はself-imageのスコアがPS群で有意に良好であった．合併症はPS群で矯正術後一過性の下肢筋力低下，肋間神経領域の知覚障害と疼痛を各1例に認めた．考察：PS法の手術成績はハイブリッド法と比較して，矯正率とその維持，self-imageの点で良好であった．さらに長期検討が必要であるが，短期の成績からみればPS法は特発性側弯症の後方手術法として有用であると考えられた．
