整形外科術後感染における術前患者背景とその対策―主に関節リウマチ患者について

　整形外科手術に限らず術後感染発症に対する術前の患者背景には種々の因子があり，その中でも生物学的製剤を使用している関節リウマチ患者や糖尿病を合併している患者の手術部位感染(SSI)には注意を要する．当科において，生物学的製剤加療中に整形外科手術を施行された28例38部位について検討したところ，3症例に術後SSI様所見を認めた．このうち2例は糖尿病を合併しており，1例は術前のインフリキシマブ休薬期間が10日であった．このような免疫不全因子が重複している症例では，なお一層SSI発症の危険性を念頭に置かなければならない．
