創傷治癒

はじめに

　1999年にCDC(米疾病予防管理センター)の「手術部位感染防止ガイドライン」^3）が発表されて以来，本邦においてもこれまでの慣例に従った創管理がエビデンスに基づいた管理へと変化している．

　従来わが国では，「創感染予防には消毒が必要」という考えが一般的で，術後であっても手術創は手術翌日から抜糸まで毎日消毒し，ガーゼ交換することが常識とされてきた．しかし，正常創は縫合閉鎖48時間後には皮膚の上皮化が完成するため，創閉鎖から48時間が経過すると創の消毒やドレッシング材は不要となる．

　また，感染などで開放された創の場合にも，創面の消毒が行われた後，ガーゼを貼付するといった創傷管理が習慣的に行われてきた．しかし，近年では創面を洗浄し，創傷被覆材などで創を閉鎖して湿潤環境を保つことにより治癒を促進させるmoist wound healing^11）という考え方が主流となってきている．

　今回，創傷および創傷治癒の分類について述べ，手術切開部位における創管理と感染発生後の急性創傷に対する管理のあり方を中心に述べる．
