膝骨壊死症の病因・病態

　膝関節にみられる骨壊死のうち，いわゆる特発性膝骨壊死の病因・病態を，軟骨下脆弱性骨折の概念に基づき再検討した．早期(Stage2)では軟骨下骨折のみを認め，骨壊死は認めなかった．進行期(Stage3)には骨壊死を認めたが，骨壊死巣はすべて骨折部より末梢側にのみ限局していた．以上の病理所見は，特発性膝骨壊死において認められる骨壊死は，軟骨下脆弱性骨折の結果であり，虚血によるものではないことを示唆している．特発性膝骨壊死は，長年にわたり虚血による骨壊死と考えられてきたが，軟骨下骨折を起因とした病態であると考えるべきであろう．
