誌上シンポジウム 膝骨壊死の病態と治療

膝骨壊死の画像診断―ステロイド性，特発性壊死の比較

　ステロイド性136例，250関節，特発性48例，50関節，計184例，300関節の膝関節部骨壊死の単純X線，MRI所見を検討した．ステロイド性は両側性，多発性の症例が多く，好発部位は大腿骨外顆後部，遠位骨幹端，内顆後部の順で，壊死発生部位は骨髄内血流終末部に一致しており，MRI所見は，「band」像，「mixed」像を呈していた．特発性は大部分が大腿骨内顆中央に限局し，MRI所見は「diffuse」像を呈し，「band」像は認めなかった．特発性壊死はステロイド性壊死と大きく異なっていたが，骨折が特発性骨壊死の原因と考えられる画像所見は認められなかった．
